How do you throw grenade VATS in Fallout 4?

You can't do it anymore, sadly. There is rank 4 of the Demolitions perk which lets you shoot bullets at grenades and when you hit they do massive extra damage. So you can throw your own grenade and target it before it goes off, or shoot an enemy grenade or mine. But sadly no, it seems like you can't use VATS to target grenades.

They basically replaced it with Rank 2 of the Demolitions perk (Perception 5 required). At Rank 2, you see an arc showing where the grenade will land. Which is handy, but it would be nice if both were possible.
