Manila, P. I., July 20, 1905. SIR: I have the honor to forward, for your information, the following statistical and sanitary report of the Board of Health for the Philippine Islands and the city of Manila for the month of June, 1905.

In beginning, I wish to state that the nomenclature of causes of death followed in this report has been made to conform with the International Manual of Causes of Death, issued by the United States Census Office in 1902. Although only minor differences have been found between the classification which has been used until now and that of the Census Office, it has been deemed advisable to adopt the latter, so that the nomenclature in use here shall conform with that used by other countries.

Five hundred and eighty-three births were registered in the city of Manila during the month, of which 295 were males and 288 females, giving a birthrate of 32.26 per thousand inhabitants. Of these, 522 were born in wedlock and 61 out of wedlock. Taking as a basis the information furnished by the certificates which were registered as to the place of birth of the children, it is found that the district of Ermita, Malate, Paco, etc., registered the largest average, and San Nicolas the lowest. The following table shows the number of births by districts:

There were 627 deaths registered, of which 593 were residents and 34 transients, the former giving a death rate per thousand of 32.81, and a daily average of 20 deaths. If this mortality is compared with that of the same month of the preceding year, 44.28, it will be found that there is a difference of -11.47 in favor of the present year. Among the causes of death registered, gastro-enteric diseases have been prevalent, especially among children under 2 years of age; 84 deaths under 2 years were caused by gastro-enteritis and by chronic diarrhea and enteritis, and 53 deaths 2 years and over by diarrhea and enteritis—19 per cent of the total number of deaths.

There were 7 cases of bubonic plague registered, followed by 6 deaths, the victims being 6 Filipinos and 1 Chinaman.

Of smallpox, there were 3 cases and 1 death.

According to the reports received of smallpox in the provinces, 1 case with recovery was registered in Occidental Negros, 4 cases in Batangas, 1 case in Tayabas, 5 cases and 1 death in Laguna, 1 case and 1 death in Cavite, and 2 cases in Masbate. Finally, in the Province of Tarlac, from the month of March until the 13th of June, 40 cases and 17 deaths have occurred from this disease.

Two hundred and thirty-six thousand two hundred units of vaccine virus were sent to 19 provinces of the Archipelago for the use of the vaccination service, and 450 units were furnished to various institutions of this city, making a total of 236,650 units distributed during the month.

Eight cases of plague, with 7 deaths, were reported to this office from the city of Cebu.

The disinfection brigade made 238 disinfections, most of which were places in a filthy and insanitary condition.

Twenty thousand and eighteen inspections and 4,633 reinspections of houses were made by the sanitary inspectors and their assistants.

Of the orders given for cleaning 1,986 yards, 1,659 have been complied with. Eighteen thousand nine hundred and sixty-nine rats were bought at the health stations.

There were 7,030 animals of various kinds and species landed at this port from abroad, and inspected by the veterinarians of this office, by whom 6,131 head of cattle were also examined at the abattoir, before slaughter for public consumption.

The inoculations of cattle against rinderpest have, as a whole, given fairly satisfactory results.

There were, during the month, 30 animals inoculated in Tuguegarao, Cagayan, with 5 deaths; 16 in Naic, Cavite, with 5 deaths, and 90 in Umalog, Misamis, with 1 death.

No report has yet been received here of the results attained in Bato, Albay, where 200 inoculations of carabaos were made. Very respectfully,

Manila, 20 de Julio de 1905. SEÑOR: Tengo el honor de remitir á V. para su conocimiento el siguiente informe demográfico-sanitario de la Junta de Sanidad de las Islas Filipinas y de la ciudad de Manila correspondiente al mes de Junio próximo pasado.

Antes de comenzar, he de manifestar á V. que la nomenclatura de causas de muerte ahora seguida en este informe se ha ajustado al Manual Internacional de Causas de Muerte publicado por la Oficina del Censo de Washington en 1902. Aunque las diferencias halladas entre la nomenclatura hasta aquí seguida por esta oficina y la últimamente citada de 1902 no existen cambios esenciales, se ha creido sin embargo conveniente adoptar ésta con el objeto de no apartarse este report de las modificaciones aceptadas en otros países de la nomenclatura internacional.

Los nacimientos registrados fueron 295 varones y 258 hembras, sumando un total de 583, dando por consiguiente una natalidad por mil habitantes de 32. 26. Son legítimos 522 é ilegítimos 61. Tomando por base los datos consignados en los certificados registrados respecto al sitio de nacimiento de las criaturas, resulta que en el distrito de Ermita, Malate, Paco, etc., ha correspondido mayor natalidad y San Nicolás la menor. ΕΙ cuadro siguiente demostrará mejor la natalidad por distritos:

Las defunciones registradas fueron 627, 593 residentes y 34 transeuntes, acusando los primeros una mortalidad por mil de 32.81, correspondiendo al tipo de 20 muertes por día. Comparada esta mortalidad con la de igual mes del año pasado, que fué de 44.28, resulta el mes objeto de este report favorecido con —11.47, mortalidad cuya explicación por causas de muerte se verá en el siguiente cuadro:

Entre las causas de muerte registradas han predominado las enfermedades gastro-entéricas, especialmente entre los niños menores de 2 años, pues la gastro-enteritis y la diarrea y enteritis crónica causaron 84 óbitos en menores de 2 años, y la diarrea y enteritis en mayores de 2 años 39, acusando un 19 por ciento de la mortalidad general.

Se registraron 7 casos de peste bubónica seguidos de 6 defunciones; fueron víctimas 6 filipinos y 1 chino. De viruela hubo 3 casos con una defunción.

Según las noticias recibidas de la viruela en provincias, en Negros Occidental se registró 1 caso sin ninguna defunción, en Batangas 4 casos, Tayabas 1 caso, Laguna 5 casos con 1 defunción, Cavite 1 caso con 1 defunción, y Masbate 2 casos. Finalmente, en la Provincia de Tarlac, desde Marzo al 13 de este mes han ocurrido 40 casos y 17 defunciones causados por esta enfermedad.

Doscientas treinta y seis mil doscientas unidades de linfa vacuna fueron enviadas á 19 provincias del Archipiélago para el servicio de vacunación, y para el mismo fin en varias instituciones de esta Capital se facilitaron 450 unidades de este profiláctico, sumando un total de 236,650 unidades las distribuidas durante el mes.

De la ciudad de Cebú se reportaron á esta oficina 8 casos de peste con 7 defunciones. La brigada de desinfección verificó 238 desinfecciones, recayendo el mayor número en sitios sucios

Se practicaron 20,018 inspecciones de casas por los inspectores sanitarios y sus ayudantes, y las reinspecciones llevadas á cabo por los mismos empleados fueron 4,633.

Se notificaron la limpieza de 1,986 patios y se limpiaron 1,659. Se han comprado 18,969 ratas en las estaciones sanitarias.

Los animales de clases y especies distintas traidos de fuera á este puerto inspeccionados por los veterinarios de esta oficina fueron 7,030, y los reconocimientos practicados de ganado destinado al matadero para el consumo público ascendió á 6,131.

Las inoculaciones practicadas en el ganado contra la epizootia han dado en general bastante halagüeños resultados.

Durante el mes fueron inoculados 30 animales en Tuguegarao, Cagayán, de los cuales murieron 5, 16 en
Naic, Cavite, con 5 muertos, y 90 en Umalog, Misamis, con 1 muerto. No se ha recibido aún parte de los resultados obtenidos en Bató, Albay, donde se verificaron 200 inoculaciones en el ganado caraballar. Muy respetuosamente,

Sir: I have the honor to forward, for your information, the following report of the Board of Health for the Philippine Islands and the city of Manila for the month of July, 1905. The report comprises the vital and sanitary statistics of this city, together with certain information concerning the public health in the provinces.

The records of this office show that 339 male and 305 female children were born in the city during the month, making a total of 644, and giving a birthrate of 34.49 per thousand inhabitants. This average plainly shows that the registration of births is far from being complete. This defect is found in other countries, even in those which are among the more highly developed, and the responsibility lies with the people themselves, who are indifferent to this important statistical measure.

The following table will show the number of births by districts:

Seven hundred and ninety-tive deaths were registered during the month in the city, of which 747 were residents and 48 transients, the former giving a death rate of 40 to the thousand. The average for the corresponding month of last year was 46.399, or 119 deaths more during July of last year. On the other hand, the average for the month of June, 1905, was 32.26, which, compared with 40 for July, 1905, gives a difference in favor of June of 7.74 to the thousand in the mortality rate.

Manila, P. I., September 27, 1905. Sir: I have the honor to submit the following report comprising the vital sanitary statistics of the city of Manila for the month of August, 1905, and also information concerning certain diseases occurring in the provinces during this period:

Before the usual explanatory statement of the contents of this report, it may be well to consider to some extent the question of the unexpected reappearance of cholera in this city and in some of the towns of the Province of Rizal.

From time to time during the past year cases have come to the attention of the Board of Health which clinically resembled Asiatic cholera, but the diagnosis of which could not be verified bacteriologically. For the two weeks preceding August 23 the number of suspicious cases was increased. One occurred at San Pedro Macati, one in a bakery in Paco, one in the San Miguel district, one case was that of a soldier in Cuartel de España, and it is now known that a number occurred in Rizal Province. It was not until August 23, however, that a case developed in Bilibid Prison, which was later found by laboratory examination to be cholera. The first victims were Patricio Lavante and Macario Alboran. On the following day six cases of suspected cholera were registered at Fort McKinley. An extra session of the Board of Health was held, after which Dr. Freer, Dr. Strong, and the undersigned, upon the invitation of the medical officer in charge, proceeded to the fort to make an investigation of the nature of the disease which in so unexpected a manner had made its appearance in that military reservation. After examining the cases it was impossible to state positively that they were cholera, although the consensus of opinion was that they were very suspicious. No material could be obtained for a microscopical examination. Several days later, however, the diagnosis was bacteriologically confirmed.

On the 25th Mrs. Rebecca Beyer, an American residing at the Grand Hotel, Walled City, was attacked, and also Mr. W. P. Lane, who had arrived in the city the night before from Bataan Province and had taken lodging at the house of his brother at No. 171 Calzada de San Sebastian. It was learned later that Mr. Lane was not absent from the city, on his Bataan trip, more than forty-eight hours. Both patients were removed to the cholera hospital at San Lázaro.

A telegram received on the 26th from Jalajala, through the Army medical department, contained the following information: “Cases of a disease resembling cholera have developed in Jalajala, the first case being registered on the 21st; from that date to the 25th 16 cases and 12 deaths have been registered, the illness lasting from twelve to fourteen hours." On that same date the provincial president of the board of health of Rizal reported, by wire from Pasig, one suspicious case, followed by death, in that town.

On the following day the death, from cholera, of a nun in the monastery of Santa Clara, Manila, was reported to this office. At that convent the most rigid regulations are observed, and communication with the outside world reduced to a minimum. As this was rather an unusual case, a full and accurate statement of the facts concerning it was secured, and, among other things, it was ascertained that it was the nun who was in charge of the infirmary who had taken the disease. The infirmary is located far inside the building, where there would be very little probability of its coming in contact with anything from the outside. The day of her death the nun felt indisposed and took a laxative from the dispensary of the convent, apparently recovering. After the noon meal she ate some lanzones, supposed to have been taken from a tree in the orchard of the convent, and a few hours later, carly in the afternoon, she was taken ill with the disease, dying between 8 and 9 of that evening, with all the symptoms of cholera. No other case occurred in the convent.

In the meanwhile other cases were being registered from nearly all the districts of the city, although the number of cases was not very great, as may be seen by the report. From the 23d to the 31st of the month only 51 cases and 46 deaths were recorded.

In making this review of the outbreak no effort has been made to draw any definite conclusions, the only aim having been to state certain facts which may be useful later in making further researches into the origin of the disease.

It having been demonstrated, clinically and microscopically, that the disease is Asiatic cholera, the question now suggests itself: Has the infection been again imported, or is it only a recrudescence of the last epidemic? This is too important a question for discussion in a letter of transmittal. After more data are obtained, and after more thorough investigation, the question may be taken up again.

The following measures were among those adopted by the Board of Health for the suppression of the disease :

(1) All possible means were taken to prevent the contamination of the city water supply. The military immediately furnished sufficient cavalry to guard against the infection of the Mariquina River above the city intake. A pail system was established where necessary by the Municipal Board, and representatives of the Board of Health were immediately detailed to proceed to the Mariquina Valley to aid the municipal officials in isolating any cases which might occur.

(2) A marine quarantine was instituted for the protection of other islands and ports.

(3) The sick were immediately isolated in the cholera hospital and the houses, contents, and contacts thoroughly disinfected.

Particular attention was given to the extermination of flies.

A campaign of education was begun at once. Circulars containing sixteen simple rules for the prevention of cholera, printed in English, Spanish, and the various native dialects, were distributed. The General Superintendent of Education issued instructions to all teachers throughout the Islands to teach the contents of the circular to the children. In addition, the church authorities sent out large numbers of circulars with the request that the information which they contained should be generally diffused.

Other measures were also taken, such as the prohibition of the sale of certain vegetables, fruits, and native sweetmeats and other articles likely to spread infection.

There were 615 births registered during the month, giving a birthrate of 32.93 per thousand. Of these, 322 were males and 293 females; 543 legitimate and 72 illegitimate. During the same period of last year only 535 births were registered.

The records of this month show that 841 deaths were registered among residents, with a death rate of 45.03, and 62 deaths among transients, making a total of 903 deaths. During the month of August, 1904, 1,032 deaths were registered among residents and 62 among transients, the former giving an average death rate of 55.28, which, compared with 45.03 for 1905, gives in favor of August of this year i0.25. The explanation of this will be found in the following table of the causes of deaths :

The number of deaths among children belonging to the groups of ages under 30 days and between 30 days and 1 year was 599 in August, 1904, representing 53 per cent of the total mortality, while the corresponding number of deaths for this month has been only 334, or 36 per cent, 17 per cent less than during the same period

No cases or deaths have been registered in the city from smallpox, and only 4 cases with 4 deaths from bubonic plague. During August of last year 4 cases and 4 deaths were registered from smallpox and 6 cases and 7 deaths from bubonic plague, one of the latter being a case of the preceding month.

Two thousand six hundred and twenty-four prescriptions were filled at the municipal dispensaries, an average of 87 prescriptions per day.

One thousand three hundred and ninety-nine visits were made by the municipal physicians to 417 patients. The medical inspectors and municipal physicians treated in the clinics 362 patients in 876 consultations. Three hundred and twenty-seven disinfections were performed in the city. Twenty-eight thousand five hundred and fifty-five rats were bought. All other sanitary work performed during the month will be found specified in the respective tables.

Two cases of smallpox with no deaths have occurred in the Province of Cagayan, 8 cases with 3 deaths in Occidental Negros, 2 cases with no deaths in Masbate, 22 cases with 11 deaths (from July 19 to August 16) in Nueva Ecija, and 5 deaths in Bagac, Bataan. The presence of smallpox in various barrios of the Provinces of Batangas, Iloilo, Sorsogon, and Capiz has also been reported.

Five cases of plague were recorded during the month in the city of Cebu.

Two hundred and thirty-two thousand seven hundred units of vaccine virus were sent to 25 provinces to carry on the vaccinations against smallpox. Very respectfully,

Manila, I. F., 27 de Septiembre de 1905. SEÑOR: Tengo el honor de poner en sus manos el siguiente informe demográfico-sanitario de la ciudad de Manila correspondiente al mes de Agosto último, así como las novedades referentes á ciertas enfermedades ocurridas en provincias durante este período.

Pero antes de comenzar la acostumbrada reseña explicativa del contenido de este informe, no está de más el considerar con algún detenimiento el hecho inesperado de la reaparición del cólera en esta ciudad y en algunos pueblos de la Provincia de Rizal.

El año pasado de vez en cuando llegaban á conocimiento de la Junta de Sanidad casos que clínicamente parecían al cólera, más estos diagnósticos no pudieron comprobarse bacteriológicamente. Durante las dos semanas que precedieron al 23 de Agosto el número de casos sospechosos fué aumentando. Uno ocurrió en San Pedro Macati, uno en una panadería de Paco, otro en San Miguel, otro en un soldado en el Cuartel de España, y ahora se sabe también que un buen número de casos ocurrieron en la Provincia de Rizal; sin embargo, no se confirmó hasta el día 23 de Agosto en el que un caso se desarrolló en la prisión de Bilibid, el cual fué más tarde comprobado como colérico por el examen hecho en el Laboratorio. Las primeras víctimas fueron Patricio Lavante y Macario Alborán. Al día siguiente en el Fuerte McKinley ocurrieron seis casos también sospechosos de cólera. La Junta de Sanidad celebró una sesión extraordinaria, después de la cual, el Dr. Freer, Dr. Strong, y el que suscribe, invitados por el médico militar encargado, nos trasladamos al Fuerte McKinley para informarnos bien de la naturaleza de aquella enfermedad que de manera insólita se presentó en aquella reserva militar. Después de examinados los casos, era imposible atirmar con seguridad que los mismos fuesen de cólera, aunque la opinión se inclinaba á creer que dichos casos eran muy sospechosos. No se pudo obtener deyecciones para hacer un reconocimiento microscópico. Después de

. unos días se comprobó el diagnóstico bacteriológicamente.

El 25 fueron atacados Mrs. Rebecca Beyer, americana que vivía en el Gran Hotel de Intramuros, y Mr. W. P. Lane, procedente de la Provincia de Bataan, quien llegó a la capital la noche anterior hospedándose en casa de su hermano en la calzada de San Sebastian No. 171. Más tarde llegó á saberse que, Mr. Lane estuvo ausente de la capital menos de 48 horas. Ambos atacados fueron trasladados al hospital de coléricos en San Lázaro.

Un telegrama recibido en esta oficina de Jalajala el 26, por conducto del servicio de sanidad militar, dice lo siguiente: "Se ha presentado una enfermedad en Jalajala parecida al cólera, el primer caso se registró el 21 y desde esta fecha hasta el 25 se registraron 16 casos con 12 defunciones, habiendo durado la enfermedad de 12 á 24 horas." En esta misma fecha el presidente provincial de la junta de sanidad de Rizal desde Pasig telegrafió un caso sospechoso seguido de defunción en aquella cabecera.

Al día siguiente se reportó á esta oficina el fallecimiento de una monja del monasterio de Santa Clara de esta ciudad á consecuencia del cólera. En este monasterio se observa una regla muy rígida y la comunicación con el mundo exterior está muy limitada. Como se trataba de un caso bastante raro, se procuró obtener una información completa y exacta del mismo, de la cual entre otras cosas se supo que la hermana enfermera fué la atacada de dicha enfermedad. La enfermería está situada en la parte más retirada del edificio y por esta circunstancia ella tuvo menos probabilidades de exponerse al contacto con nada que haya venido de fuera.

En el día de su fallecimiento la monja se sintió algo indispuesta y tomó un laxante del botiquín del monasterio, que al parecer la mejoró de su indisposición. Después de la comida del medio día comió algunos lanzones cogidos de un árbol que tenían en la huerta y transcurridas algunas horas, en las primeras de la tarde, fué atacada del mal y falleció entre 8 y 9 de la noche con todos los síntomas del cólera. Después de este caso no volvió á registrarse ningún otro caso en el monasterio.

Mientras tanto siguieron registrándose casos en casi todos los distritos de esta ciudad, sin demostrar mucha difusibilidad, de tal modo que desde el día 23 al 31 del mes solamente se registraron 51 casos con 46 defunciones.

Al referir el caso que antecede no se ha abrigado la idea de deducir ninguna conclusión concreta, si no simplemente el dejar consignados ciertos hechos que puedan algún día ser de alguna utilidad en investigaciones ulteriores respecto al origen de la enfermedad.

Una vez admitida que la enfermedad es el cólera asiático, comprobado clínica y microscópicamente, ocurre preguntar: ¿Ha sido importada, ó es solamente una recrudescencia de la epidemia pasada? Cuestión es ésta demasiado importante para tratada en los límites de una carta de remisión. Quizá con más datos y más amplias investigaciones, esta pregunta pueda ser considerada.

Entre otras medidas que la Junta de Sanidad puso en práctica para extirpar el mal, están las siguientes:

(1) Evitar por todos los medios posibles que el abastecimiento de aguas de la ciudad se contamine. Los militares facilitaron inmediatamente suficiente caballería para impedir la infección del río de Mariquina más arriba de la traida de aguas. La Junta Municipal instaló cubetas en donde fué necesario, y se enviaron repreentantes de la Junta de Sanidad al valle de Mariquina inmediatamente para auxiliar á los funcionarios municipales en aislar cualesquiera casos que ocurriesen.

Manila, P. I., October 19, 1905. Sir: I have the honor to submit the following report of the operations of the Board of Health for the month of September, 1905:

The records of this office show that 692 births were registered during the month, or an average of 38.29 per thousand. Of the above number 368 were males and 324 females. Six hundred and twenty-two were born in wedlock and 70 out of wedlock, the latter representing 10.11 of the total births.

There were 1,068 deaths registered during the month, of which 55 were transients and 1,013 residents, the latter giving an annual mortality rate of 56.06 to the thousand. The number of deaths during the month of

. September, 1904, reached 1,146, or 78 more than during the same period of this year.

The most prevalent causes of death during the month covered by this report were, in the order of their frequency: Convulsions ( under 5 years), 245; tuberculosis of the lungs, 128; Asiatic cholera, 126; diarrhea and enteritis (2 years and over), 72; meningitis, 58; acute bronchitis, 56; beri-beri, 38; dysentery, 36; congenital debility, icterus, etc., 31; chronic diarrhea and enteritis (under 2 years), 29; chronic bronchitis, 25; senile debility, 20; diarrhea and enteritis (under 2 years), 17; tetanus, 14; organic disease of the heart, 13; malarial fever, 12; chronic rheumatism and gout, 8; broncho-pneumonia, 8; acute nephritis, 8; chronie nephritis, 7; congestion and hemorrhage of the brain, 7; intestinal parasites, 6; typhoid fever, 5; tuberculosis of the meninges, 5; angina pectoris, 5; leprosy, 4; pneumonia, 4; puerperal hemorrbage, 4; accidental traumatism, 4; accidental electrocution, 4; malarial cachexia, 3; purulent infection and septicemia, 3; tuberculosis of the abdomen, 3; cancer and other malignant tumors of other organs not specified, 3; anemia, chlorosis, 3; acute endocarditis, 3; pulmonary congestion, 3.

There was one case of smallpox, with no deaths registered during the month. There were two cases of bubonic plague, with two deaths.

There is very little to be added to what has already been said in the letter of transmittal for the last month regarding cholera in the city. On September 1 the Bureau of Health was requested to assume charge of the medical and sanitary department of Bilibid Prison, there having been, up to this time, 27 cases of cholera in this institution. All necessary measures were immediately taken to prevent the spread and eliminate the sources of infection. The ward containing the cholera patients, and the closets, were protected by screens, so that it was impossible for flies to assist in spreading the disease. These measures were carried into effect at once, and completed September 7, after which no further cases of cholera occurred in Bilibid Prison.

Cholera in the provinces continued to spread, especially in Rizal. Isolated cases have also occurred in ('avite, Pampanga, Bulacan, and Laguna, the numbers being stated in the respective tables. Additional representatives of the Bureau of Health were sent to various towns of the Province of Rizal to coöperate with and advise the municipal authorities in the work of suppressing the epidemic.

Two deaths from old age were registered during the month: Cándida Basa, a Filipina, residing at 206, Calle Carlos IV, Paco, who reached the age of 105 years, and Máxima de los Santos, Filipina, who died in the district of Tondo, aged 10:3 years. The cause of both deaths was senile debility.

The following deaths were due to external causes: Ignacio Infantado, a Filipino, 10 years of age, residing in the barrio of Santa Clara, district of Sampaloc, was killed by a blow on the neck from a beam of his house, when the latter was blown down during the typhoon. Fausta Hermosado, 25 years of age; Alfonso Sobradilla, 34 years, a policeman on duty in the district of Tondo; Hermógenes Alcántara, 17 years; and Vicente Rivera, 20 years, were killed on the day of the storm by coming in contact with fallen electric wires. Severina Bartazan died from the effects of a severe contusion of the lumbar region. Leopoldo Lara, 23 years of age, was killed by the kick of a horse in the abdomen. Sin Chan, aged 41 years, died from fracture of the skull with hemorrhage. Two children, Enrique Pintado and Constancia Jimenez, aged 5 and 3 years, respectively, died in consequence of extensive burns. Ines ('arlos, aged 4 years, living in casco No. 2434, was drowned in the Pasig River, and a woman, whose unidentified body was found near the breakwater, was probably another victim of the storm.
